The word "serendipity" was entered into the lexicon by Horace Walpole in 1754. He had become intrigued with a Persian fairytale in which three princes of Serendip, (formerly also Ceylon, now Sri Lanka) traveled the world, "making discoveries, by accidents and sagacity, of things they were not in quest of."

The Oxford English Dictionary defines serendipity as "the faculty of making happy and unexpected discoveries by accident." Serendipity plays an important part in research of all kinds, but it operates only in a special environment; as Pasteur famously stated, "Chance favors the prepared mind." In research, what serendipity really means in practical terms is that scientists discover things in the course of their investigations that they were not looking for. Occasionally, serendipity plays a big role in a key discovery.

One excellent example of a serendipitous observation which led to a great discovery occurred in 1895, when Wilhelm Roentgen, working in his darkened lab with a Crooke’s (cathode ray) tube, noticed out of the corner of his eye that several feet away, a piece of paper coated with barium cyanoplatinate was glowing faintly. He was puzzled, since the only conceivable source of energy in the room was the tube. When subsequently Roentgen found that sealed photographic plates in his desk had become fogged in the absence of a visible light source, he deduced that a novel form of radiation was being generated in the Crooke’s tube. He termed the new radiation X-rays. Within a year after this discovery, X-rays were being applied in diagnostic medicine.

More recently, the multi-billion dollar biotechnology industry in great measure found its origins in a spontaneous, serendipitous detour:

"It would sound reasonable if I were to say that the research work…began as a result of a grand design, with a vision of the goals in mind. Unfortunately, this would not be true. This work began the day I took a detour through Yellowstone National Park on my way to Seattle." (Thomas Brock)

On this, his first visit to Yellowstone, Brock became intrigued with the multi-colored algae mats in the hot springs, and on a whim, took some samples back to analyze in his laboratory. In 1969, Brock and Freese reported the discovery of Thermus aquaticus; this bacterium became one early source from which the heat-stable enzymes were purified that are the key tools in recombinant DNA technologies.

And the pharmaceutical industry has benefited many times from serendipitous observations. Perhaps the best-known contemporary case is that of Viagra, which was originally tested as a treatment for angina. It was almost immediately found to be less effective than nitroglycerine for coronary artery dilatation, but then the patients in the first clinical trial reported an unusual, not at all undesirable and now well-known side effect. It is no wonder that the patients became depressed when the first clinical trials were brought to an end, and it was requested that the unused pills be returned to Pfizer. Pfizer noted that never had so many unused clinical trial pills been reported as "lost, misplaced, or accidentally flushed down the toilet…"

Serendipity still plays a major role in discovery and invention. It is the manifestation of inspiration, and of being in the right place at the right time. To some, it has a certain magic about it that suggests predetermination or intervention by the supernatural, or as Shakespeare wrote: "There is a tide in the affairs of men, which, taken at the flood, leads on to fortune…" (Brutus to Cassius, in Julius Caesar)

In the end, though, probably the best way to sum up the phenomenon was thoughtfully stated by Julius Comroe: "Serendipity is jumping into a haystack to search for a needle, and coming up with the farmer’s daughter."

Been a long time since a treatment update. I’ve got plenty of pictures, I just haven’t been in the mood to sort through, edit and post them. Tonight is the night before my last chemo infusion, so I thought that merited a little extra effort.

I’ve had two treatments since my last update, and just one left to go. The second round of side effects wasn’t quite as bad as the first, or it didn’t feel as bad because I knew what to expect. As you can see, I’ve lost more of my hair, including some thinning of my eyebrows, but these last bits are stubborn and won’t fall out. I think that I’ve only had to shave my face twice since starting chemo a couple months ago. In this picture it’s been 2 1/2 weeks since my last shave, and it seem like I have less than a day’s worth of stubble.
Due to a scheduling conflict, I had an extra week of rest since my last treatment so I’m feeling pretty tip top right now. The side effects were particularly bad following the last one, so despite it being my last treatment, now that I’ve had some extra time not being full of poison, I’m really not looking forward to another round of side effects.

I guess a week or two more of side effects won’t be too bad. The main thing I’m not looking forward to is the combo of nausea + hypersensitivity to smells. Not to be too gross, but I’ve been enjoying being able to pee without the smell of my own urine inexplicably triggering the taste of a mouthful of pills.

Dock’s, a fixture on Broadway, at 89th Street for the past 23 years, fell victim to the recession. Back in December of 2008, the restaurant displayed a large sign in the window (shown here) for what it called "half-price Tuesdays." It would be one thing if they offered the "house wine" for half-price during a "happy hour" period; but the sign seemed to suggest that you could order a really good hundred-dollar bottle of wine on Tuesday evenings, and save yourself …

But it apparently wasn’t enough; the restaurant closed down at the beginning of March, 2009. This new sign explains the closure; you can see the details here.

These are photos that I began taking in mid-December of 2008, after the government helpfully informed us that the recession (a word they had dared not use before, just as they refuse to use the word "depression" now) has actually been in effect since December 2007.

Having lived through roughly half a dozen previous recessions, with nothing but vague memories to mark their existence, I thought it would be useful to start gathering some photographic memories of the current one. It may or may not turn out to be as severe as the Great Depression of the 1930′s, and it may or may not produce the kind of grim photographs that we have of that era … but we won’t really know until it’s over, and our 20-20 hindsight might be better informed with some pictures.

Thus far, I have only a few pictures of signs showing steep discounts; these are interesting, but it’s the "human story" that I think will tell the more important story. Thus far, I’ve noticed an increase in the number of homeless peoplehe street, asking for money, food, and handouts; but I have no direct evidence that it’s associated with the recession itself. In any case, I’m reluctant to take advantage of such people by taking casual photos of them; I’m sure more appropriate opportunities will present themselves, and I’ll add them to the album as I find them…

Thalidomide – preparation of unusually wide margin of safety.

Thalidomide and Noxodyn is the Swedish brand name for sleeping agent and the sedative Thalidomide. In Denmark it is sold under the name Neosedyn and in Norway, the name Nevrodyn. Thalidomide was included as number 3294 of the pharmacy list of standard packaged drugs, the 1959 edition.
Starting in early 1959 sold the preparation of two degrees of strength, and tablets of 25 mg, and tablets of 100 mg. In April 1959, it makes a liquid version, thalidomide Mixture 100 ml, containing approximately 0.5 g of Thalidomide, ie the same amount as 5 x 100 mg tablets. Later, bottles of 300 ml of oral solution, a quantity equivalent to 15 tablets of the stronger sort of pills. Thalidomide-preparations are described in advertising as the most perfect sedative and hypnotic drug, virtually free of side effects.
Astra describes in a leaflet 1960 thalidomide as "a preparation of unusually wide margin of safety".

In autumn 1961 starts strong suspicion of Thalidomide-preparations teratogenic effects that are reported from several parts of the world. In Australia, an obstetrician named Mc Bride to mothers of three children extremity to which he was delivered, eaten Thalidomide tablets and no other medications during pregnancy. He informs in writing Grünenthal about this, but get no response. Then three more children with similar injuries were born at the same hospital, Mc Bride publishes his experiences in the international medical journal "The Lancet".

Thalidomide is a big seller for Grünenthal. In 1961 sales of Grünenthal thalidomide to a sale of a total of 12.4 million DM. Add to this the value of the exports, amounting to at least a quarter of the quantity sold in West Germany. Consumption this year is estimated at 1.3 tons per month.

Shortly after the Thalidomide goes on sale in West Germany, noted pediatrician, an alarming increase in the number of newborn babies with malformed limbs, sometimes accompanied by heart defects and abnormalities of internal organs. Around the same time as Mc Bride, in September 1961, draws attention to the German doctor Wiedemann, the new and seemingly widespread epidemic syndrome that has emerged in many places. The number of children born with severe extremities has increased to a level far beyond what had previously been average.

On October 27, 1961, the journal Die ärzliche Mitteilungen des Bundesärtzekammers, which goes out to 80,000 West German physician, in an article entitled "Conterganskador of the nervous system," explained the damage to the nervous system that, when studies at the University Clinic of Cologne reached.

Under hösten 1961 börjar starka misstankar om Thalidomid-preparatens fosterskadande effekter att inrapporteras från flera håll i världen. I Australien finner en förlossningsläkare vid namn Mc Bride att mödrarna till tre extremitetsskadade barn som han förlöst, ätit Thalidomid-tabletter och inga andra mediciner under graviditeten. Han informerar skriftligen Grünenthal om detta, men får inget svar. Sedan ytterligare tre barn med liknande skador fötts vid samma sjukhus, publicerar Mc Bride sina erfarenheter i den internationella medicinska tidskriften "The Lancet".

Thalidomid är en storsäljare för Grünenthal. Under 1961 omsätter Grünenthal thalidomid till ett försäljningsvärde av totalt 12,4 miljoner DM. I take a few dozen pills at various times throughout the day. It can be difficult to keep track of it all. I wake up and must eat food first for the dexamethasone. That’s an artificial steroid (actually glucocorticoid steroid) that keeps the swelling in my brain down. It’s the most important med in my fold-out aluminium case. It’s also the most side-effect packed – among other things it’s weakened my leg muscles, left me bloated and overweight and disrupted my sleep intensely. And I have to take other pills to protect my stomach and my bones from its effects.

A major knock-on of the weight gain seems to be back problems on top of myopathy (the degradation of my muscles). So I’m on a lot of painkillers. Oxycontin, Pregabalin and good old paracetamol. I take a lot and still need Oxynorm for ‘breakthrough pain’ – the stuff that wakes me in the night.

There’s the anti-seizure pills too. I’ve been having little seizures once in a while as a follow-up to December’s Grand Mal which left me on life-support for a short while. These ones aren’t very bad, and often I have an ‘aura’ or flashing lights, so I take a lorezapam under the tongue immediately. And prophylactically, I take clobazam and Phenytoin. Susan also carries a bottle of midolazam and an oral syringe in case I have another Grand Mal seizure.

Susan has created a spreadsheet of times and doses of all of these and more. She’s made labels for all the compartments in the open-out case. And she co-ordinates between the hospice, the GP and the chemists when it all runs out.

And so here I am. My morning dose of dexamethasone, omeprazole and paracetamol, washed down with movicol powder (did I mention the digestion gets screwed up too?). I’m in the middle of my first Temadol chemotherapy cycle. In 10 days, I will be adding four more pills a day. Temadol, aka Temazolomide, a chemo drug that will hopefully shrink my 19 (last count) brain lesions. I take that drug for 5 days along with some very strong anti-nausea pills. Then, wait two weeks and we have an MRI look at my brain. See what the Gamma Knife Surgery and two rounds of chemo have achieved.

For the shot, I put up one of my Esprit Gemini 250ws strobes with a 40cm Beauty Dish on it to the front right and a medium round, silver reflector propped up to the left. I put my M9 with the Voigtlander 35mm f1.2 Nokton Aspherical on a tripod in front of me and put it on self-time. The shots looked well-light on the camera display, but were dark in Lightroom, so I adjusted accordingly. I had Diego Velazquez in mind in composing, lighting and post-processing. I hope this comes across somehow.

So I started Chemo on Thursday. So far I’ve been feeling pretty ok, a little bit nauseated, but the various meds have been keeping it pretty much under control.
I have been feeling suddenly tired on and off, and one unlisted side-effect is frequent uncontrollable hiccups. Any of the normal methods that typically make hiccups go away don’t seem to work. Not that I haven’t been trying, which means I’ve been drinking tons of water, which I’m supposed to be doing anyway. My pee is very clear.
I haven’t been eating all that much, but I noticed this morning that my weight is up a bit. Then I realized that 3 of the 5 pills I’m currently on are constipators, so I’m carrying around about 2 days worth of poop.

Clear Pill? [clever viral ad]

Saw this advert for a sort of ‘miracle’ pill in a Tube carriage earlier today.

What intrigued me was the possible side effects, which the advert stated WILL (with emphasis on ‘will’) include:
* Paralysis
* Amnesia
* Extreme sexual appetite
* Brain damage
* Irreversible coma
* Homicidal blackouts
* Psychosis
* Sudden death

Hmm yes, I don’t think I’ll be trying this pill anytime soon.

Edit: apparently this ad is a spoof for a film (‘Limitless’)… Now that I notice Bradley Cooper and the ‘enter the code’ bit at the bottom of the ad it’s blindingly obvious…

When I was a kid, I was sick for about one year with a deadly kidney infection. I was in the hospital for many months, and a lot of that time was in the intensive care. The life revolved around taking more than 20 pills a day, in addition to very painful intravenous medications. Being surrounded by patients all of who were fighting to survive, and many were not that lucky, has left a very deep impact on my psyche.

Medicines saved my life, like they have done more and more effectively for humanity. And at the same time, they are a part of my nightmares.They are also despised globally for the side-effects.
